1. Do you have pain behind your eyes?

2. Do you experience pain or stiffness in neck?

3. Do you get frequent headaches?  Especially in the morning.

4. Is there congestion, hearing loss or ringing in your ear? 

5. Have you had any pain in or around the ear?

6. Do you have nausea?

7. Are you having dizziness (vertigo)?

8. Have you had tingling in back of your hand or fingers?

9. Is there pain in area of your temples?

10. Do you experience pain or stiffness in shoulders?

11. Have you noticed popping or grating in either jaw joint?                                                                                                               
12. Are your teeth sensitive to temperature?
